Policy for Credit Card Use
To further insure customer security, we have established
the following credit card use policies:
  • We will manually verify all customer credit card numbers and addresses for validity.
  • We will aggressively follow up on any suspected credit card fraud cases with the appropriate authorities, including police and Internet Service Providers.
  • We reserve the right to require a signature and a copy of your credit card and ID for verification purposes if we deem necessary for any reason.


Credit Card and Identification Verification Form
The following procedure has been established to protect us,
and our customers, from credit card fraud:
  • Please provide the following information via facsimile
  • A signature, a copy of your credit card and a copy of your photo ID is required for verification. This is to prevent the purchase of products with your credit card without your prior knowledge and consent. You may either cut and paste a copy of your cards from a copy machine, or make two copies of this form, one for each card.
  • We have provided space for this information below. Print out this page for use in your copier, or send a separate sheet, with the copy of your ID on it, along with this page. (click here to print only this form)
